The first form becomes part of the long-term record.
The accident description should be factual and concise. The symptom and employment sections should be complete without guessing at diagnoses or permanence.2
The insurer may request additional disability, employer, treatment or expense forms. Each one addresses a particular benefit question and should be reviewed in that context.
Use a simple application sequence.
1. Give notice
Report the claim promptly and obtain the insurer’s claim number and contact information.
2. Complete the OCF-1
Answer accurately, identify other insurance and attach what the current form requests.
3. Build supporting proof
Coordinate medical, employer, treatment and expense records as the claimed benefits require.
Create an application record you can audit later.
Save the final signed form and every page before sending it.
Keep portal receipts, email confirmations, fax reports or courier tracking.
List first care, later providers, referrals, diagnoses and material changes.
Record information requests, examinations, approvals, denials and reasons by date.
Do not wait for perfect medical certainty before giving notice.
Prompt notice protects the process while the diagnosis develops. If information is not yet known, say so accurately and update the insurer when reliable information becomes available.3
A denial should identify reasons and review rights. Calendar the applicable dispute deadline rather than assuming discussion with the adjuster stops time.
