The employer supplies facts; the insurer applies the benefit rules.

Confirm employment dates, job title, essential duties, schedule, earnings periods, absences and return attempts. Variable income may require more than one pay period.2

The claimant’s authorization should be read for scope. Keep the final employer form and any supporting payroll material provided.

02 · Insurance and responsibility

The OCF-2 supports several income questions.

Employment status

Was the person working and on what terms?

Earnings

What source records support the reported income?

Absence and return

When did work stop or resume, and under what accommodations?

03 · Evidence

Reconcile the form before submission.

01Payroll

Pay stubs, ledger and earnings breakdown.

02Schedule

Regular, overtime, seasonal and variable hours.

03Job demands

Actual physical, cognitive and travel requirements.

04Other income

Bonuses, commissions, benefits and collateral sources.

Use payroll, schedules and tax records instead of rounded estimates from memory.
04 · Timing and decisions

Correct errors in writing with the supporting record.

Do not alter an employer’s signed form yourself. Ask the employer to correct or explain inaccuracies.3

A calculation or entitlement denial may create LAT timing even while payroll questions continue.
