One headline number rarely describes the whole file.

Match each OCF-18 or expense to the insurer’s approval or denial.
Reconcile billed, approved, paid and disputed amounts without assuming they are identical.
Do not wait until a limit is nearly exhausted to organize the ledger.
Regularly compare proposed care with approvals, payments and the insurer’s stated remaining balance. Challenge unexplained entries promptly.3
